RECORDS OFFICE POLICIES AND PROCEDURES
STUDENT REGISTRATION
New students may be registered in the Attendance Office located on campus from 7:30 – 3:00 P.M. Parents must complete the enrollment application, registration packet, provide a Legal Birth Certificate, custody paperwork or proof of guardianship (if applicable), immunization records, previous school records (grades, attendance, discipline) and proof of residency (two forms of proof). If you reside outside of our District, or outside of Hosler boundaries, you will need to obtain a Permit from the District Office; before you can obtain an enrollment packet and/or enroll at our School.
EMERGENCY CARDS
Parents/guardians of each student will be required to fill out a Pupil Emergency Card. Students may only be release during the school day to those adults who are named on the Emergency Card. The person who is requesting that the student be released to them must also present a valid California identification card to school personnel. There are no early dismissals the last 15 minutes of every day, and students will not be released during lunch.
CHECKING OUT OF DISTRICT PROCEDURES
Please return Text Books, Library Books and Computer to the Library - You will be provided with a receipt for the returned books and/or fines paid to the Library. Pay any money owed for Candy/ Fundraisers etc. to the Student Store or Teacher in charge of the club. Bring documents and Student ID to the Attendance Office to process the dropping of the Student. You will be asked for ID, to verify you are the parent or guardian on file. At that time, you will be given a copy of Demographic page, Grades, Transcripts, Discipline, Test Scores, Health Record which is to be given to the next school of enrollment.
VERIFICATION OF ENROLLMENTS
Parent/Guardian can request Verification of Enrollment by coming to the Attendance Office and providing their valid Identification - Please note, verification will be given with the names provided on birth certificate and/or guardianship paperwork and addresses that are in aeries.
RECORDS REQUESTS
Request for Records must be requested from the New School.
