Student Records & Enrollment

Below you’ll find all the information you need regarding student enrollment, emergency procedures, and record requests. If you have questions, our Records Office is here to help.

Student Registration

New students may register in the Attendance Office on campus from 7:30 a.m. to 3:00 p.m.

To enroll, parents/guardians must complete the Enrollment Application and Registration Packet, and provide the following:

  • Legal birth certificate

  • Custody paperwork or proof of guardianship (if applicable)

  • Immunization records

  • Previous school records (grades, attendance, discipline)

  • Two proofs of residency

If you live outside of Hosler boundaries or outside Lynwood Unified School District, you must first obtain a permit from the District Office before enrolling at Hosler.

Emergency Cards

Every student must have a Pupil Emergency Card on file.

  • Students may only be released during the school day to adults listed on the Emergency Card, who must show valid ID.

  • No early dismissals will be allowed during the last 15 minutes of the school day.

  • Students will not be released during lunch.

Withdrawing from School / Out-of-District Checkout

Families checking out of Hosler must complete the following steps:

  1. Return all textbooks, library books, and Chromebooks to the Library and pay any outstanding fines (a receipt will be issued).

  2. Pay any outstanding balances (e.g., fundraiser or club fees) to the Student Store or teacher in charge of the club.

  3. Bring your ID to verify you are the parent or guardian on file, as well as the student’s ID, to the Attendance Office to complete the withdrawal process.

  4. Upon verification, you will receive a copy with:

    • Demographic information

    • Grades and transcripts

    • Discipline history

    • Test scores

    • Health records

These documents must be submitted to the student’s next school.

Verification of Enrollment

Parents/guardians may request Verification of Enrollment by visiting the Attendance Office with valid ID. Please note:

  • Only names listed on the birth certificate or guardianship paperwork will be used.

  • Address must match what's listed in Aeries.

Student Records Requests

If your student is enrolling in a new school, the new school must request records directly from Hosler Middle School. Records will not be released to individuals or families.
